I agree with Sureshot. The normal reply would be your #3. “I’m fine, thank you.”

Your #1 can also be used in some cases. Suppose someone comes up to you and asks, “How are you?” and hands you a gift. You might reply, “I’m fine. Thank you.” or even “I’m fine; thank you.”

The difference between the two uses of “thank you” is the difference between the senses “thank you for asking” (which is what #3 says) and “thank you for the gift” (which is what #1 says).
